We do not believe in “wait and see.” Wounds can go bad fast, so our first priority is getting back to you immediately. When you contact us, you won’t get lost in a corporate voicemail loop. The first thing that happens is you get a phone call from our office. We need to know what is happening right now. If our nurse practitioners are out in the field and can’t get to you physically that same instant, we don’t leave you hanging. We often jump on a Zoom call. This allows our specialists to put “eyes on the wound” immediately, assess the urgency, and give you peace of mind while we schedule the physical visit.
2. Telehealth vs. Home Visits: We Come to You
For many of our patients, just standing up or getting into a car is a suffering. We have seen spouses struggle to pack wheelchairs and walkers into vans just to sit in a waiting room for hours. It is stressful and exhausting. We are a mobile wound care clinic. That means the hospital-grade care comes to your bedroom or living room.

- The Clinical Assessment: We look at the wound with clinical expertise (our lead NP is Mayo Clinic trained). We determine if you need advanced treatments like sharp debridement or collagen dressings.
- The Environmental Scan: We look at your surroundings. Are there barriers to movement? What food is in the kitchen? (Yes, we look at the donuts!).
- The Team Huddle: We don’t work in a silo. We collaborate with your primary care physician, your home health nurses, and your family. You need a team in your court to heal.
4. Closing the Gap: Continuity of Care
The biggest cracks in the healthcare system happen during transitions like moving from a hospital to a nursing home, or from a nursing home to your own house. We bridge that gap.
We follow you. If you are discharged from a facility, we can follow you home to ensure your care plan doesn’t fall apart.
We also train your “home team.” Whether it is a spouse or a caregiver, we ensure they know exactly what to do from nutrition plans to moving the lymphatic system, so you aren’t left guessing between our visits.
